Outreach services (includes gardening)
Community Aged Care Packages (CACP) and Extended Aged Care at Home (EACH) are available for ACAT assessed and program eligible persons. CACPs and EACH are planned and coordinated packages of community care services, to help people with complex care needs remain living in their own home. Care needs can include personal care, help with shopping and housework, attending medical appointments or simply catching up with friends and family.
