Outreach services (includes gardening)
Community Care Packages and EACH are available for ACAT assessed and program eligible persons. CCPs and EACH are planned and coordinated packages of community care services, to help people with complex care needs remain living in their own home. Care needs can include personal care, help with shopping and housework, attending medical appointments or simply catching up with friends and family.
Charges to clients funded through the CCP program are $15 for six hours or less per week, $20 for more than six hours, including gardening. Unfunded clients are charged $5 per hour for care and $11 per hour for gardening.
Charges for EACH and TCP clients are $44 per week, including gardening.
